Mu Alpha Theta Math Honorary Membership Application

Mu Alpha Theta is a mathematical honor society whose main goals are to inspire keen interest in mathematics, develop strong scholarship in the subject, and promote the enjoyment of mathematics in high school and two year college students.

Membership Requirements:

Full members are high school students in grades 9 through 12, whose permanent record resides at Ashland Paul G. Blazer High School, who have completed four semesters of college preparatory mathematics and have completed or are enrolled in a fifth semester of mathematics, provided that this work has been done with distinction. On the 4-point grading scale, this shall mean at least a 3.0 grade point average.

Associate members are high school students in grades 9 through 12, whose permanent record resides at Ashland Paul G. Blazer High School, who have completed two semesters of algebra or their equivalent with distinction and who have completed, or are enrolled in, a semester of geometry or second-year algebra. Associate members do not pay the initiation fee. They are entitled to attend and be heard at meetings of the chapter, but are not entitled to vote on questions of national policy. Associate members planning to attend the National Convention or compete in national competitions must be registered with the National Office.

Members shall demonstrate ability to work with others and to conduct independent research, and shall possess qualities of industry, initiative, and reliability. No student will be denied membership on account of race, religion, color, creed, ethnicity, national origin, gender, sexual preference and/or physical disability.

Selection of members shall be made by the Admissions Committee (See Article 7) composed of the sponsor and the officers of the chapter.

Attendance requirements for continued membership in good standing are 4 of the listed monthly meetings. Each member should attain 10 hours of participation through meetings, fundraising, and community involvement. Certainly it is intended that all members take an active part in the chapter.

Return to Mrs. Stout (Room 210) by September 30th. You will be notified of your acceptance after grade verification. Upon Acceptance, you will owe a $25 membership fee the first year (incudes registration and T-shirt) and each year following you must pay a $5 club fee to remain in the club. A mandatory induction ceremony will be held on Sunday October 16th at 2:00 in the cafeteria.
